Overview
In this role you will deliver patient-centred clinical pharmacy care on the ward, optimizing inpatient treatment and medicines safety. You will work with ward clinicians, practice pharmacists, GPs and carers to support evidence-based prescribing and improve outcomes across the care pathway. Your leadership on medicines optimisation and contribution to governance will help raise standards in the ward setting. Responsibilities
- Conduct comprehensive inpatient medication reviews to optimise therapy and outcomes
- Provide expert prescribing advice to clinical and non-clinical colleagues
- Lead medicines optimisation initiatives to maximise patient benefit
- Promote safe, effective, and cost-conscious medicine use in line with formulary
- Identify unmet pharmaceutical needs and uphold medicines safety and governance
- Undertake deprescribing to reduce unnecessary medicines and waste
- Provide ward leadership on medicines-related matters, including risk management and governance
- Support safe and timely discharge through medicines reconciliation and team coordination
- Deliver patient-facing care and educate patients and carers to support adherence
- Educate and train ward staff on medicines optimisation and related practices
