HCRG Care Group is leading the delivery of a Virtual Ward model to prevent avoidable hospital admissions. This role is based at Peasedown St John but works across community services and people's homes. Service hours: 8am‐8pm, seven days a week. The Advanced Clinical Practitioner will lead assessment, management and treatment of patients in community settings, initially focusing on referrals from the 2‐hour Urgent Community Response (UCR) and developing wider referral pathways.
Main Responsibilities
- Provide supervision, support, professional development, appraisals and rota management for an 18‐person multidisciplinary team.
- Deliver evidence‐based advanced nurse practice, including assessment, diagnosis, interpretation of tests, and forward planning of care & treatment.
- Recognise deteriorating patients promptly, respond appropriately, and document escalation.
- Write drug charts and prescribe medication for patients in the Virtual Ward and in preparation for discharge.
- Initiate and participate in audit & research to improve current practice, drive continuous improvement, and contribute to knowledge development.
- Identify poor practice and support improvement opportunities through SBAR, thematic review, and a Just Culture approach.
The Ideal Candidate
- Registered with the NMC or HCPC and educated to degree level.
- Recognised teaching or mentorship qualification.
- Current Non‐Medical Prescriber qualification.
- Significant experience in elderly care, frailty, community services or relevant acute care speciality.
- Proven leadership and team‐management experience.
- Master's degree.
- Clean driving licence and access to a car.
