You will be based at Livingstone Hospital, Dartford, North Kent, as part of our community nursing team providing care to people in their own homes or within a clinic setting.

As this is a community-based role, you will need a UK driving license and access to a car.
Main Responsibilities
Working closely with our Community Nursing Team, as a Healthcare Assistant, you will support senior team members in providing care to patients in their homes or a clinic setting for conditions including diabetes, wound care, and end-of-life care. Our work is vital to the community, aiming to prevent unnecessary hospital admissions. You will deliver high standards of care, share best practices, and actively seek to improve our services.
The Ideal Candidate
This role is ideal for someone with NVQ Level 3 or equivalent qualifications/experience. Strong communication and organizational skills, along with a flexible approach, will help ensure positive patient experiences throughout their healthcare journey.
Travel across the area is required, so a UK driving license and access to a car are necessary.
You should also be confident using IT systems and have a working knowledge of Microsoft Office applications such as Outlook, Word, and Excel.
