HCRG Care Group is seeking a Band 7 Speech and Language Therapist to join the Early Supported Discharge Stroke Team at Gravesham Community Hospital, Gravesend.

You will deliver specialist assessment, diagnosis and rehabilitation for adults with communication and swallowing disorders as they transition from hospital to home.

You will work autonomously within a multidisciplinary team, develop personalised care plans, support discharge planning and contribute to service development.
