As a Band 6 Occupational Therapist, you will join our Early Supported Discharge Service based on Sapphire Ward at Gravesham Community Hospital. This is a rewarding opportunity to support patients recovering from stroke and neurological conditions as they transition from hospital to home. You will play a key role in delivering high quality, patient centered rehabilitation, helping people regain independence and confidence in their daily lives. This is a varied role that combines inpatient work with community visits, giving you a unique opportunity to follow patients through their recovery journey. This is a full time role working Monday to Friday, 8am to 4pm.
Main Responsibilities
In this role, you will deliver specialist Occupational Therapy assessment and intervention for patients with stroke and a range of neurological conditions. You will support early discharge from hospital, ensuring patients receive the right care at the right time in the most appropriate setting. You will work across both the inpatient setting on Sapphire Ward and within patients’ homes, supporting rehabilitation in real life environments. This approach allows you to make a meaningful impact on patient outcomes, helping individuals regain independence as quickly and safely as possible. Alongside your clinical work, you will support junior colleagues, students and therapy assistants, contributing to the development of the wider team. You will also play an active role in service improvement, working with senior therapists to enhance and evolve the Occupational Therapy provision.
