Overview
As part of our commitment to your lifelong learning we offer the care certificate to all our unregistered health care professionals. This is the start of your education and training with us and forms part of a robust induction programme giving you the fundamental evidence-based training to support you in your role with us. The Care Certificate is an agreed set of standards that define the knowledge, skills and behaviours we expect of specific job roles in health and social care services. Working closely with our Intermediate Care Team, as a Community Rehab Assistant you will support and assist the senior team members to provide care and rehab to people in their own homes. Our work is crucial to the community as we aim to prevent unnecessary hospital submission. In this role, you will care for patients in the way that you believe is best, delivering our high service standards, sharing best practice, and actively improving the way we work. As part of our community nursing team we empower our staff to work autonomously and with flexibility by providing them with state of the art new tablets for them to be able to check in from home, write their notes up on the go and allow them to not be trapped in an office base, so they can really enjoy working in their chosen community background
Note: The content includes information about a part-time role and shift pattern. The role is described as 28.5 hours per week with early or late shifts (7.30–3.30 or 1–9pm) covering a 7-day period. These shifts are shared equally between the team.
Responsibilities
- To assist in providing rehabilitation and care in patients’ own homes, following a care plan designed with and for the patient, following illness or a hospital admission.
- Assist with home exercise programmes as prescribed by the therapy team.
- Work alongside other agencies (e.g., Hospice Team, District Nursing Team, Adult Social Care Reablement Team) to deliver joint rehab or end of life care.
- Maintain knowledge, skills and attitudes necessary to deliver a quality service that is responsive to the changing needs of patients.
- Provide clerical and administrative support to the team.
- Deliver care in accordance with organisational policies, procedures and guidelines, according to competencies, as delegated by OPS Lead/Therapist.
- Report any changes in condition of the patient to the Operational Lead.
This role provides an opportunity to apply NVQ Level 2/3 or equivalent qualification/experience. Strong communication and organisation skills, and a flexible approach, will help you deliver positive experiences to patients at every stage of their healthcare journey.
Qualifications and Requirements
In this role, you will need to be able to drive and have access to your own car to travel across our services in a timely manner. You should be confident using IT systems and have working knowledge of Microsoft Office packages (Outlook, Word, Excel) and EMIS.
