We are seeking an experienced and motivated

Transition Lead Nurse (Band 7) to lead a high-profile programme focused on improving the experience and outcomes of young people transitioning from children's to adult health and care services.

This is an exciting opportunity to drive transformational change across a complex system, working in partnership with health, social care, education, voluntary sector and independent organisations .

As the

Transition Lead Nurse , you will be responsible for developing, implementing and embedding integrated transition pathways, policies, Standard Operating Procedures (SOPs), and best practice guidance that promote consistent, safe and person-centred care.

The post holder will work collaboratively with a wide range of stakeholders to reduce variation in practice, strengthen partnership working, and ensure young people receive coordinated support as they move between services .

You will provide leadership and expert guidance to professionals across organisations, helping to establish clear frameworks that support effective transition planning and improve long-term outcomes .

The successful candidate will have significant clinical experience, excellent project management and stakeholder engagement skills, and a passion for improving services for young people.

This role offers a unique opportunity to leave a lasting legacy by shaping and embedding sustainable transition arrangements that place young people and their families at the centre of care.

  • This part time role is fixed term for 12 months
  • Main Responsibilities
  • Lead the development and delivery of an integrated transition programme to improve outcomes for young people moving from children's to adult services .
  • Review current transition arrangements, identify opportunities for improvement, and develop standardised pathways aligned with national guidance and best practice.
  • Design, implement and embed transition pathways, policies, Standard Operating Procedures (SOPs), and supporting tools across partner organisations .
  • Collaborate with young people, families, carers and professionals to ensure transition services are person-centred, inclusive and responsive to individual needs .
  • Build strong partnerships across health, social care, education, voluntary and independent sector organisations to drive service improvement and shared accountability .
  • Chair and facilitate multi-agency stakeholder groups, providing leadership, coordination and expert advice on transition best practice.
  • Develop and monitor performance measures, analyse outcomes and implement quality improvement initiatives to reduce variation and improve the experience of transition.
  • Deliver training, guidance and resources to support practitioners in understanding and fulfilling their roles within transition pathways .
  • Provide regular progress updates to senior leaders and governance groups, ensuring sustainable improvements are embedded beyond the lifespan of the project.
  • Act as a specialist clinical resource and advocate for excellent transition planning, promoting safe, effective and coordinated care for young people

Essential Qualifications

  • Registered Nurse (NMC Registered).
  • Degree in Nursing or equivalent professional qualification.
  • Evidence of continuing professional development.
  • Significant experience working with children, young people or adults with complex health needs.
  • Experience of leading service development or quality improvement projects.
  • Experience of multi-agency and partnership working.
  • Experience influencing change across organisational boundaries.
  • Strong project and programme management skills.
  • Excellent stakeholder engagement and relationship-building skills.
  • Ability to analyse complex systems and develop practical solutions.
  • Excellent written communication skills, including report and policy writing.
  • Ability to facilitate workshops and collaborative working groups.
  • Strong understanding of change management principles

Other requirements: Full UK Driving Licence and access to a car insured for business use.
