This post is to join a team of existing Brain Injury Case Managers at CA Case Management Ltd. CA Case Management Ltd is a case management company specializing in the provision of high-quality case management services to clients with brain and spinal cord injury and long-term neurological disabilities.

Under the direction of Brain Injury Case Managers, the CMA is responsible for advocating, planning, and facilitating the delivery of complex rehabilitation and care/support to enable clients to achieve their maximum potential. The CMA will be responsible for building a relationship with our client and those important to them, as well as working alongside the case managers, homecare team, litigation solicitor and/or deputy, and as part of bespoke MDTs. Working to agreed standards and in line with codes of professional practice; supporting our case manager and homecare team in order to provide specialist clinical and rehabilitation, and care and support services for our clients.

Key Responsibilities:

  • Supporting the management of the client’s home including adaptations, equipment, maintenance and cleaning
  • Setting up and monitoring MDT
  • Supporting the client’s Case Management goals and rehabilitation
  • Supporting production and proof reading of assessment, reports, meeting minutes
  • Support clients to manage and attend clinical and rehab appointments
  • Making applications on behalf of the client e.g. blue badge, DWP form completion
  • Supporting and supervising the care teams
  • Completing homecare visits and checks
  • To visit the client and support meetings as required
  • Work in accordance with relevant legislation, health care best practices and internal policy and procedures.
  • Be responsible for ensuring that clients’ files are charged and invoiced appropriately in-line with agreed working practices and contractual customer agreements.

Person Specification:

Experience

  • Minimum 5 years experience within a CQC regulated service
  • Experience working as part of a team delivering complex care or therapy
  • Experience working with Case Managers
  • Experience of working with adults and children with neurological conditions, traumatic brain injury / Cerebral Palsy and other complex conditions

Skills & Knowledge

  • A caring and empathetic individual
  • Full, clean driving licence and access to a car for work
  • Excellent administrative skills
  • Good working knowledge of Microsoft Word is required, including formatting documents, using templates, and editing content
  • Initiative and problem-solving skills, with the ability to work independently and seek solutions using available resources
  • Ability to work quickly and accurately through a range of tasks, setting appropriate priorities and meeting deadlines
  • Excellent written and verbal communications skills
  • A willingness to go ‘above and beyond’ for the client
  • Ability to research or find points of contact within both private and statutory services
  • Accuracy in file notes, data entry and message taking
