- Graduate in Physiotherapy, Sports Therapy or Sports Rehabilitation, post-graduate clinical hands-on experience required, relevant industry background preferred

Providing services to the insurance, corporate and legal sectors, HCML assists injured and ill individuals to recover and return to work or their pre-injury condition. An exciting opportunity has arisen in our team for a Rehabilitation Case Manager with a background in an MSK Therapy discipline – for example, Sports Therapy or Sport Rehabilitation. HCML’s Corporate Health Rehabilitation Case Managers utilise their clinical experience in a telephonic and video-based environment to effectively assist injured individuals with their recovery (return to function and work).
This role requires the applicant to possess high levels of communication to influence and support a patient’s journey, whilst maintaining great attention to detail within a fast-paced environment; often with competing priorities. This role would suit somebody who is looking to utilise their clinical skillset and experience outside of the hands-on clinical or sporting environment.

The day-to-day role includes:
- remotely assessing patient needs using biopsychosocial model;
- handling calls from patients, customers or treatment providers;
- managing outcomes from HCML digital rehabilitation services;
- managing an active open casebook;
- supporting Customer Experience Consultants with capturing patient and customer feedback;
- using experience and expertise to deliver excellent patient and customer-centred outcomes.
Main Roles and Responsibilities:
Clinical Service Delivery
- Deliver clinically robust, consistent, and comprehensive remote (phone or video) assessments which lead to clinically excellent outcomes
- Assess, plan, implement and evaluate appropriate interventions and clinical pathways throughout the case leading to expedient patient recovery and case closure
- Regular and consistent use of the bio-psychosocial model with patient case management and with clinical decisions
- Utilising expertise at every contact point with patients as a chance to educate & provide clinical support
- Capture and routinely assess progress against set goals of rehabilitation, working collaboratively with treating practitioners to achieve functional outcomes
Patient Care
- Individual and shared responsibility for patient cases, i.e. owning a caseload and supporting colleagues in a team focused environment
- Contribute to key performance metrics that enable HCML services to thrive
- Adopting and encouraging the highest standards of patient care, including the consistent provision of clinical guidance and accurate (and maximising use of systems/platforms available to you)
- Taking a forward thinking, patient-centred approach to overcoming case hurdles
Personal Profile:
- Relevant clinical degree (sport therapy, sport rehabilitation, or physiotherapy) ~1+ years clinical/hands-on experience or previous case management or vocational rehabilitation expertise
- Current governing registrations of your relevant professional body e.g. BASRaT or SST, HCPC body (or able to quickly obtain membership)
- Live within travelling distance to an HCML office – Croydon / Manchester (hybrid working setup) (Preferred)
- Experience with delivering high quality customer service and case management (Preferred)
- Experience in Health, Insurance, Medical industries (Preferred)
- Experience of investigating and resolving complaints, knowledge of FCA (Financial Conduct Authority) regulations
