HC-One is seeking a Care Team Leader to oversee a care team in a home setting. The role is full-time (44 hours per week) and reports to the Home Manager.
You will lead by example, ensure residents receive person-centred care, and uphold values of fairness, respect and integrity. You will work as part of a committed team, with emphasis on communication, teamwork and quality care delivery for residents with diverse needs, including dementia care.
