Overview
HC-One has developed a Specialist Dementia Care Community (SDCC) at Meadow Bank care home. Sabrina House has provided care for people living with dementia for many years and, following investment in environmental layout and design, workforce and L&D, we have exciting opportunities for those passionate about supporting people who are emotionally distressed by their dementia and require a specialist service. Sabrina House SDCC is a therapeutic care setting that demonstrates that quality of care and quality of life are of equal significance. It will provide a bespoke living environment embracing space and calming de-escalation rooms, along with enhanced staffing and specialist training for colleagues in positive approaches to behaviour, psychosocial therapies, de-escalation interventions and safe restrictive practices. The therapeutic ambition is that people will be understood as having needs to be met, rather than defined by their condition. Personalised lifestyle support plans are delivered through a person-led, strengths-based approach with a focus on life story work. This role promotes curiosity, radical kindness and compassion, and the attitude of never giving up.
Responsibilities
As a Therapeutic Wellbeing Practitioner, you will:
- Support residents to live each day in a meaningful and fulfilling way after getting to know them and the significant people in their lives.
- Deliver life skills and tailored activities programmes focused on stimulating the body, mind and soul, and personalise residents’ rooms to create a “room as your home”.
- Collate and use life story work, maintain and encourage interests, preserve independence where possible, and engage residents in spontaneous meaningful moments of conversation and fun.
- Lead the effective delivery of ongoing assessment, planning, implementation, review and evaluation of wellbeing activity, using a bespoke Wellbeing Assessment Tool to support meaningful lifestyles.
- Attend Case Review Meetings to report on the review of Personal Wellbeing Plans and wellbeing activity.
- Inspire colleagues to participate in daily wellbeing activities and support residents to live the life they want while receiving the care they need.
- Be a role model to colleagues, promoting wellbeing as a core part of everyone’s job.
Qualifications and Experience
We are looking for someone with a background in working with vulnerable individuals, ideally developing activity programmes, and with knowledge and experience of dementia care and wellbeing. The successful candidate will value meaningful occupation and social interaction, and bring a positive energy with the ability to communicate with residents, colleagues, families and external stakeholders with kindness and consideration.
