Your new role
We are seeking a dedicated and motivated Recovery Coordinator to join our multidisciplinary team. This is a rewarding and dynamic role supporting individuals throughout their treatment and recovery journey. You will play a key part in reducing drug and alcohol-related harm, promoting recovery, and helping people rebuild meaningful, independent lives. As a Recovery Coordinator, you will offer high-quality, person-centred support from first contact through to ongoing recovery, while working collaboratively with a wide range of partner agencies, clinicians, volunteers and peer mentors.
Key Responsibilities
- Supporting Service Users
- Provide support from point of entry into the service and throughout each person's treatment and recovery journey.
- Carry out screening, triage assessments, comprehensive assessments, and develop individual recovery plans.
- Coordinate personalised packages of care including psychosocial interventions, 1:1 keywork, group work, clinical pathways, and harm minimisation interventions.
- Help individuals strengthen their recovery capital and engage with peer support, mutual aid groups and community resources.
- Work creatively to improve engagement, particularly for under represented groups.
- Care Coordination & Multi-Agency Working
- Act as the main care coordinator for a caseload of service users.
- Liaise effectively with mental health teams, clinicians, GPs, hospitals, social care, community organisations and family/carers.
- Provide advocacy to ensure service users can access partnership services and wider support.
- Accurately assess suitability for groups based on risk and individual needs.
- Harm Reduction & Health Interventions
- Support clinical teams in substitute prescribing pathways.
- After training, assist with DBST, saliva/urine testing, needle exchange, and the distribution of Naloxone.
- Service Contribution
- Support the smooth day to day running of the service, including duty cover, reception, and occasional clinical support tasks.
- Ensure timely and accurate recording, data collection and reporting in line with service requirements.
- Uphold confidentiality, professional boundaries and information governance standards at all times.
- Promote carer, community and service user involvement in shaping recovery services.
