Job Description

Your new companyAn exciting opportunity has opened up to join Birmingham City Council's Transitions & Preparation for Adulthood Service as a Grade 5 Social Worker.

Your new roleWe're looking for qualified Social Workers with 2+ years' post-qualification experience who are passionate about supporting young people (aged 17-25) with additional needs as they transition into adulthood. This includes working with individuals facing challenges such as homelessness, mental health issues, and involvement in the criminal justice system.

Key Responsibilities:

  • Conducting Conversation 3 discussions
  • Support planning & commissioning care (budget requests)
  • Reviews & mental capacity assessments
  • Attending multi-agency meetings (Best Interest, EHCP, Section 117)
  • Completing CHC checklists

You'll be supported by experienced Senior Practitioners and a dedicated Team Manager.Hybrid Working: 1 day/week at Sparkbrook Community Health Centre ??35 hrs/week | 3-month rolling contract ??Up to Grade 5 – Β£33.33/hr

  • Recognised Social Work qualification (MA/BA/B.Sc., DipSW, CQSW)
