This role is to support individuals with complex needs, including homelessness and co-existing mental health and substance misuse challenges.

Location: Barnet, north-west London

Dates: asap - for 3 months (Temporary Contract, will be reviewed after 3 months)

Working days: Monday to Friday

Your new role:

  • Conduct physical psychosocial assessments for individuals experiencing homelessness, using advanced nursing skills.
  • Provide BBV screening and support integrated care pathways for Hepatitis B, Hepatitis C and HIV.
  • Administer Hepatitis A/B vaccinations where clinically indicated.
  • Work collaboratively with community teams and voluntary organisations.
  • Act as an advocate for people experiencing homelessness, promoting independence, empowerment, and self-care.
  • Undertake phlebotomy, interpret results, and support decisions around detox suitability and specialist referrals.
  • Refer to inpatient detox services and liaise with hospital teams as required.
  • Support the Naloxone Strategy by training staff and service users and ensuring wide distribution of naloxone.
  • Provide wound care assessments and basic dressings.
  • Deliver harm reduction and health promotion interventions.
  • Carry out specialist mental health assessments for dual diagnosis service users.
  • Provide case management for identified dual diagnosis clients.

Essential Qualifications and Experience:

  • Current NMC registration
  • Experience in substance misuse and/or mental health settings
  • Experience working with people with co-morbidities
