Your new role: This is a temporary Tenancy Support Worker role, providing flexible cover across services when needed. You will work alongside people experiencing housing or wellbeing challenges, offering person led, trauma-informed support that empowers them to create meaningful, sustainable change.
Your responsibilities will include:
- Carrying out needs assessments to understand each person's circumstances, strengths, and goals.
- Co-producing Individual Support Plans that promote independence and reflect personal aspirations.
- Reviewing support plans and risk assessments to ensure they remain relevant and empowering.
- Working collaboratively with local partners, housing providers, and community organisations.
- Offering advice and guidance through community drop-ins across Blaenau Gwent.
- Coordinating long-term and crisis interventions, including attending joint meetings and case reviews.
- Supporting people to access information, digital tools, and resources to manage housing, benefits, and wellbeing.
- Providing resettlement support for those moving into new or independent accommodation.
- Maintaining accurate case notes and sharing information appropriately with colleagues.
- Providing flexible outreach support in people's homes and within the community.
