You will be joining an established organisation which places the service user at the heart of everything they do. This organisation is focused on supporting vulnerable people in the community, primarily those whose lives have been held back by a range of social deprivation.
The post holder will work in the community as part of a multidisciplinary team, and in close partnership with rough sleeping and street-based services, to proactively engage with individuals with physical and/or mental health issues involved in drug and alcohol use and rough sleeping.
- To co-ordinate and lead a proactive solution focussed approach to clients with a view to engaging them in appropriate services, moving them away from rough sleeping, and preventing their return to street-based living.
- To identify and establish contact with people who rough sleep and/or beg, working within protocols to rapidly assist service users through street outreach shifts at a variety of times and a range of locations.
- To identify and establish contact with individuals who have experienced multiple exclusion homelessness rough sleep and have been rough sleeping for a long term, including those who have not been able to previously engage with services, and provide them with outreach services.
- To carry a case load of complex clients and develop personalised action plans (identifying goals, aspirations and recovery focused interventions) that ensure clients are discouraged from rough sleeping.
Must have worked in an 'Outreach worker role' previously and also worked for a substance misuse service.
