Your new company Birmingham City Council are looking to recruit a qualified Social Worker for their Discharge to Access (D2A) Service. The team supports the assessment and support planning for citizens who have left the hospital with a short-term service and require support to determine the longer term support needed where applicable. There is also a subteam which covers Summer Hill Terrace, which is part of the Homeless Pathway.
Your new role involves working as part of the Discharge to Assess Community Social Work Team to deliver on-person-centred practices where citizens are enabled to exercise choice, control, independence, and build resilience with a focus on prevention and early intervention to support, provide guidance and signposting in addition to assessments, via the 3 Conversations model, in accordance with the Care Act 2014.
Homeless Pathway cases can be quite complex as the support in Summer Hill Terrace deals with complex housing issues relating to substance misuse issues, safeguarding issues and working in a very multidisciplinary setting. You will also have extensive knowledge of social work practice and legislation, including the Care Act 2014, Safeguarding, Mental Health and Mental Capacity Act 2005, a good understanding of personalisation and excellent assessment and analysis skills.
Working Hours: 8:30am-5pm (Mon-Thurs) and 8:30am - 4:30pm (Friday).
