Your new company Sandwell Council is looking for two experienced and motivated Social Workers to join our Acute Hospital Team based at the Midland Metropolitan University Hospital (MMUH). You will play a pivotal role in supporting safe, effective hospital discharge planning, preventing delays, and ensuring residents receive the right care at the right time.
Your new role As part of a fast-paced, multidisciplinary environment, you will work closely with NHS colleagues, therapy teams, and community services to deliver high-quality, person-centred interventions.
Key Responsibilities- Complete strengths-based assessments and discharge planning in line with the Care Act 2014.
- Undertake mental capacity assessments (MCA) and contribute to Best Interest decisions.
- Work collaboratively with hospital teams to ensure timely and safe discharge pathways.
- Identify risks and put in place appropriate safeguarding measures.
- Work directly with adults, families, and carers to promote independence and wellbeing.
- Maintain accurate case records and ensure compliance with statutory duties.
- Contribute to multi-disciplinary team meetings and joint care planning.
You will be working in a friendly environment, with a skilled, diverse and supportive team.
What you'll need to succeed- Have a recognised professional Social Work qualification (MA/BA/BSc in Social Work, Diploma in Social Work or CQSW)
- Social Work England registration
- 3 years post-qualifying experience.
- Previous experience working within Social Work adults services
- Strong communication and interpersonal skills.
