Your new company Birmingham City Council have an excellent opportunity for an experienced Senior Social Worker to join their Transitions & Preparation for Adulthood Service. Your new role This role supports young people within Birmingham aged 17-25 with additional needs as they transition into adulthood. The caseload may also include individuals facing challenges relating to homelessness, mental health, and involvement with the criminal justice system. Key responsibilities include: ✔ Support planning and undertaking Conversation 3 discussions ✔ Completing commissioning of care forms and budget requests ✔ Conducting reviews and Mental Capacity Assessments ✔Attending Best Interest Meetings, EHCP meetings, and Section 117 reviews ✔Completing CHC checklists as part of case management ✔ Making sound, defensible decisions in a fast-paced environment You will receive ongoing support from experienced Senior Practitioners and the Team Manager.
