Are you passionate about improving health and wellbeing beyond traditional medical interventions? Do you believe that strong communities, meaningful relationships, and access to local resources can have a profound impact on people's lives? We are looking for an enthusiastic and compassionate Social Prescriber or Health and Well-being Coach to join our practice team. This is an exciting opportunity to work directly with individuals who may be experiencing social isolation, poor mental wellbeing, financial hardship, housing difficulties, long-term health conditions, or other challenges affecting their quality of life.

As a Social Prescriber/Health and Well-being Coach, you will provide personalised, non-clinical support, helping people identify their strengths, build confidence, and access community-based services and activities that support their health and wellbeing goals. You will work as part of a multidisciplinary team alongside GPs, nurses, pharmacists, a mental health practitioner, voluntary sector organisations, and local community groups to deliver holistic support that addresses the wider determinants of health.

Main duties of the job

You will manage a varied caseload of individuals referred through the practice team. Using a person-centred approach, you will spend time understanding what matters most to each individual, helping them identify achievable goals and connecting them to services and opportunities that can improve their wellbeing and independence. Examples of support may include:

  • Reducing loneliness and social isolation.
  • Improving mental health and emotional wellbeing.
  • Supporting people with long-term health conditions.
  • Accessing welfare benefits, debt advice, and financial support.
  • Improving physical activity and healthy lifestyles.
  • Supporting carers and families.
  • Addressing housing, transport, or practical barriers.
  • Connecting individuals to local groups, activities, and peer support networks.

Job responsibilities

  • Receive and manage referrals in line with practice procedures.
  • Undertake comprehensive wellbeing conversations and holistic assessments.
  • Co‑produce personalised wellbeing plans with individuals.
  • Provide ongoing support, motivation, and follow‑up as required.
  • Support individuals to build resilience, confidence, and self‑management skills.
  • Maintain professional boundaries while developing trusting relationships.
  • Develop and maintain strong links with community, voluntary, and statutory organisations.
  • Map and promote local services, activities, and opportunities.
  • Identify gaps in community provision and share intelligence with service partners.
  • Encourage community participation and social inclusion.
  • Work collaboratively with healthcare professionals and multidisciplinary teams.
  • Attend case discussions, team meetings, and relevant partnership forums.
  • Contribute to integrated approaches that improve population health outcomes.
  • Act as an advocate for individuals when appropriate.
  • Maintain accurate and timely records using electronic systems.
  • Ensure compliance with GDPR, confidentiality, safeguarding, and information governance requirements.
  • Collect outcome measures and contribute to service evaluation.

Qualifications

  • Personalised Care Institute certification
  • Certificate in Social Prescribing

Experience

  • Working in health, social care, community development, housing, welfare support, mental health, or a related field, supporting vulnerable adults or individuals with complex needs.
  • Experience building partnerships whilst managing a caseload and prioritising workload effectively.
  • Has understanding of person‑centred and strengths‑based approaches, with knowledge of local community resources and voluntary sector services.
  • You must be an excellent communicator and possess active listening skills, with an ability to motivate and empower individuals to achieve their goals.
  • Strong organisational and time‑management skills.
  • Ability to maintain accurate records and produce reports.
  • You will also need to be able to work independently as well as part of a team, with an empathetic and non‑judgemental approach and a flexible, adaptable style.
  • Experience working within primary care.
  • With relevant certification or related discipline.
  • Personalised Care Institute accreditation.
  • Training in motivational coaching, behaviour change, or mental health awareness.
  • Knowledge of community asset‑based approaches.
  • Understanding of population health management principles.
