Main duties of the job

Main Duties and Responsibilities

The successful candidate will be expected to independently manage and review patients across a range of long-term condition disease registers, including:

• Diabetes
• Asthma
• COPD
• Coronary Heart Disease
• Chronic Kidney Disease
• Stroke/TIA
• Heart Failure
• Learning Disabilities reviews
• Other long-term conditions as appropriate

Additional responsibilities will include:

• Conducting annual reviews and monitoring of patients with chronic diseases.
• Developing and implementing personalised care plans.
• Undertaking clinical assessments and examinations.
• Prescribing medications within competence and scope of practice.
• Reviewing and adjusting treatment plans in line with national and local guidelines.
• Delivering health promotion and preventative healthcare interventions.
• Vaccinations and immunisations.
• Cervical cytology
• Supporting Quality and Outcomes Framework (QOF) achievement as well as local and national incentives.
• Maintaining accurate clinical records and coding.
• Participating in practice clinical audits and service improvement initiatives.

Job responsibilities

Main Duties and Responsibilities

The successful candidate will be expected to independently manage and review patients across a range of long‑term condition disease registers, including:

• Diabetes

• Heart Failure
• Learning Disabilities reviews
• Other long‑term conditions as appropriate

Additional responsibilities will include:

• Qualified and competent in the fitting, removal, and management of Long‑Acting Reversible Contraception (LARC), including contraceptive implants (Nexplanon/Implanon) and intrauterine contraception (coils).
• Evidence of relevant accredited training and maintenance of competency in LARC procedures.
• Conducting annual reviews and monitoring of patients with chronic diseases.
• Developing and implementing personalised care plans.
• Undertaking clinical assessments and examinations.
• Prescribing medications within competence and scope of practice.
• Reviewing and adjusting treatment plans in line with national and local guidelines.
• Delivering health promotion and preventative healthcare interventions.
• Vaccinations and immunisations.
• Cervical cytology.
• Supporting Quality and Outcomes Framework (QOF) achievement as well as local and national incentives.
• Maintaining accurate clinical records and coding.
• Participating in practice clinical audits and service improvement initiatives.
• Registered Nurse (NMC registered).
• Experience working within General Practice or Primary Care.
• Proven experience managing long‑term condition clinics and disease registers.
• Strong clinical assessment and decision‑making skills.
• Excellent communication and interpersonal skills.
• Ability to work autonomously and as part of a multidisciplinary team.
• Proficient in clinical IT systems and electronic patient records.
• Chronic Disease Management qualifications.
• Cervical Cytology qualification.
• Experience of EMIS Web.
• Mentor or teaching qualification.

• Applicants must be able to demonstrate the following:
• Registered Nurse with current NMC registration.
• Substantial experience working within a General Practice or Primary Care setting.
• Proven experience in the autonomous management of long‑term conditions and disease registers.
• Ability to undertake comprehensive clinical assessments and formulate appropriate treatment plans.
• Experience in managing patients with chronic diseases including:
• Diabetes
• Asthma
• COPD
• Coronary Heart Disease
• Chronic Kidney Disease
• Heart Failure
• Stroke/TIA
• Ability to prescribe safely, effectively, and within scope of professional competence.
• Experience in the management and monitoring of patients receiving anticoagulation therapy, including interpretation of INR results and adjustment of treatment in accordance with local and national protocols, or willing to train towards this.
• Demonstrable competence in the assessment, monitoring, and ongoing management of patients prescribed warfarin and other anticoagulant therapies.
• Knowledge of current NICE guidelines, QOF requirements, and chronic disease management frameworks.
• Experience of undertaking annual reviews and monitoring for patients with long‑term conditions.
• Excellent communication and interpersonal skills with the ability to build effective relationships with patients, carers, and colleagues.
• Strong organisational skills with the ability to prioritise workload and work independently.
• Commitment to delivering patient‑centred care in line with the practice ethos and values.
• Ability to work collaboratively as part of a multidisciplinary team.
• Competent use of clinical IT systems and accurate clinical record keeping.
• Understanding of clinical governance, safeguarding, infection prevention and control, and health and safety requirements.
• Evidence of continuous professional development and commitment to maintaining clinical competencies.

• Previous experience as a Nurse Prescriber within General Practice.
• Spirometry qualification and interpretation skills.
• Diabetes Diploma or equivalent chronic disease management qualification.
• Experience using EMIS Web.
• Immunisation and vaccination training.
• Experience supporting QOF achievement and quality improvement initiatives.
• Mentorship, teaching, or supervision qualification.
• Experience participating in audits, service development, and workforce training.
