We are seeking a dedicated and proactive Targeted Youth Support Case Support Worker to assist in delivering targeted interventions for vulnerable young people and their families. This role is essential in preventing risks from escalating and ensuring young people receive the right support at the right time.
Key Responsibilities:
- Support case management for vulnerable young people aged 11-19 (up to 25 with SEND).
- Assist in conducting Early Help Assessments (EHA) and developing intervention plans.
- Coordinate and provide administrative support for Team Around Family (TAF) meetings.
- Maintain accurate case records and update management information systems.
- Work closely with multi-agency professionals, including social care and youth justice teams.
- Provide advice, guidance, and signposting for young people and their families.
- Help track and monitor the progress of intervention plans.
- Support service development by identifying areas for improvement.
- Ensure safeguarding policies and procedures are followed at all times.
- Participate in relevant meetings, supervision, and training sessions.
Skills & Experience Required:
- Experience working in a social care or youth support setting.
- Knowledge of Early Help, safeguarding procedures, and risk management.
- Strong communication and interpersonal skills.
- Ability to manage workload, prioritise tasks, and meet deadlines.
- Good IT skills, including the ability to maintain accurate records.
- Ability to work collaboratively across agencies and services.
- Understanding of equality, diversity, and the importance of inclusive practice.
