Practice Nurse (Part time)

We are looking for a highly motivated, friendly and organised Practice Nurse with a keen eye for detail who wants to make a difference to patients, and who is qualified in managing Long Term conditions, mainly patients with Asthma and COPD. We are looking for someone for 16 hours per week, who will, at all times, deliver the high level of evidence-based individual care to the Practice population.

The post holder will have the responsibility for the provision of nursing services related to General Practice which will include participating in the assessment of patient care which will include long term conditions management, treatment, preventative care, screening and patient education. The post holder will work at times without supervision.

The post holder will provide management/health education for Asthma, COPD and CHD. Prescribing desired but not essential.

• Pathological Specimens and Investigative Procedures

Undertake the collection of pathological specimens including intravenous blood samples, swabs etc. Perform investigative procedures requested by GPs. Attend and participate in Practice meetings as and when required.

We have a fantastic team here at Haslingden which comprises of 2 GP partners, 1 salaried GP, 3 x regular locum GPs, 1 ANP, 2 Practice Nurses, plus numerous additional roles staff including a First Contact Physio, Trainee Associate Psychological Practitioner, Physician Associate, Pharmacist, Pharmacy Technician and Health and Wellbeing Coach. Our admin team comprises of a Practice Manager and deputy together with many supportive staff including reception. We are keen to support learning and encourage development within our staff.

• Ensure confidentiality at all times
• Ensure accurate and legible notes of all consultations and treatments are recorded in patients notes
• Ensure the clinical computer system is kept up to date with accurate details recorded
• Attend and participate in Practice meetings as and when required
• Report any problems to the Practice Manager

The post holder will be involved in the maintenance of chronic disease registers. The post holder will evaluate the outcome of care, making changes to care plans for individual patients and modifying their own practice with the patient.

Training requirements will be monitored by annual appraisals and will be in accordance with Practice requirements and developments. The post holder will complete annual statutory/mandatory training. The post holder will develop and maintain an in-house training programme reporting to the Nurse Team Leader. Personal Development plans will be encouraged and supported by the Practice. It is the individual's responsibility to remain up to date with recent developments.

The post holder may have access to confidential information relating to patients and their carers, Practice staff and other healthcare workers; they may also have access to information relating to the Practice such as a business organisation. All such information from any source should be regarded as strictly confidential.

The post holder will strive to maintain quality within the Practice and will:

• Alert other team members to issues of quality and risk

• Contribute to the effectiveness of the team by reflecting on their own and the team activities and making suggestions on ways to improve and enhance the team performance

The post holder will ensure the adoption of safe working practices consistent with the Practice Health & Safety policy to include:

• Identify the risks involved in work activities and undertaking such activities in a way that manages risk
• Using appropriate infection control procedures, maintaining work areas in a tidy and safe way and free from hazards
• Reporting patients risks identified
• Responsible for the correct and safe management of specimens including collection, labelling, handling, use of correct and clean containers, storage and transport arrangements
• Management and maintenance of Practice medical equipment including provision, ordering, availability and ongoing correct usage by staff
• Safe management of sharps procedures e.g. storage and disposal

Qualifications: Diploma in Asthma and COPD. Disclosure and Barring Service Check. This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
