Harwood Group Practice is excited to invite an Advanced Nurse Practitioner to join our friendly and forward-thinking team. This full‑time position (37.5 hours per week) operates between 08:00‑18:30, Monday to Friday.
Your time will be split between providing high‑quality care within the practice and conducting home visits for our more vulnerable patients in the community who are unable to attend the surgery.
Key Responsibilities- Autonomous Care: Assess, diagnose, and treat patients with a wide range of acute and chronic conditions.
- Community Outreach: Provide essential clinical support through home visits, ensuring continuity of care for housebound patients.
- MDT Collaboration: Work closely as part of a diverse Multi‑Disciplinary Team, including GPs, nurses, and allied health professionals, to deliver integrated patient care.
- Clinical Excellence: Maintain high standards of prescribing and clinical decision‑making to improve patient outcomes.
- Assess and treat same‑day urgent conditions such as respiratory infections, abdominal pain, skin infections, and minor injuries.
- Chronic Disease Management: Proactively manage long‑term conditions, adjust medication regimes, and prevent hospital admissions.
- Home Visits: Lead the clinical response for housebound patients.
- Autonomous Consultations: Conduct face‑to‑face, telephone, and video consultations to triage, assess, and diagnose patients.
- Physical Examinations: Perform comprehensive physical examinations.
- Independent Prescribing: Issue prescriptions safely and effectively in accordance with scope of practice and local formularies.
- Diagnostic Requests: Order and interpret clinical tests, including bloods, X‑rays, and ultrasounds, and act promptly on the results.
- Referrals & Signposting: Make appropriate referrals to secondary care or specialist community services.
- MDT Participation: Attend daily clinical huddles or MDT meetings to discuss complex cases and safeguarding concerns.
- Clinical Administration: Manage own clinical workflow, including processing clinical letters, actioning lab results, and maintaining accurate records via EMIS.
- Health Promotion: Provide opportunistic screening and health advice to empower patients to manage their wellbeing.

Qualifications & Experience- Essential: Registered Nurse (NMC) with Independent/Supplementary Prescriber (V300) and MSc in Advanced Clinical Practice.
- Experience: Proven track record as an Advanced Nurse Practitioner in primary care with autonomous prescribing.
The post requires a Disclosure to the Disclosure and Barring Service under the Rehabilitation of Offenders Act (Exceptions Order) 1975.
