Harwood Group Practice is excited to invite an Advanced Nurse Practitioner to join our friendly and forward-thinking team. This full‑time position (37.5 hours per week) operates between 08:00‑18:30, Monday to Friday. Your time will be split between providing high‑quality care within the practice and conducting home visits for our more vulnerable patients in the community who are unable to attend the surgery.
Key Responsibilities
- Autonomous Care: Assess, diagnose, and treat patients with a wide range of acute and chronic conditions.
- Community Outreach: Provide essential clinical support through home visits, ensuring continuity of care for housebound patients.
- MDT Collaboration: Work closely as part of a diverse Multi‑Disciplinary Team, including GPs, nurses, and allied health professionals, to deliver integrated patient care.
- Clinical Excellence: Maintain high standards of prescribing and clinical decision‑making to improve patient outcomes.
Main duties and daily responsibilities
- Assess and treat same‑day urgent conditions such as respiratory infections, abdominal pain, skin infections, and minor injuries.
- Chronic Disease Management: Proactively manage long‑term conditions, adjust medication regimes, and prevent hospital admissions.
- Home Visits: Lead the clinical response for housebound patients.
Daily tasks and responsibilities
- Autonomous Consultations: Conduct face‑to‑face, telephone, and video consultations to triage, assess, and diagnose patients.
- Physical Examinations: Perform comprehensive physical examinations.
- Independent Prescribing: Issue prescriptions safely and effectively in accordance with scope of practice and local formularies.
