Harrow Council is seeking a dedicated Team Manager for a newly remodelled Young Adulthood (18-25) team. You will lead social workers, occupational therapists and care managers to deliver person-centred, evidence-based care within the Mental Health, Learning Disability and Autism service.
You will embed the Transitions Framework, start planning before a child’s 18th birthday, and work across Adults and Children’s directorates to meet statutory obligations under the Care Act 2014.
