You will assess and develop care plans to promote independence and safety from first contact, and coordinate adaptations and equipment with a focus on rapid response and early intervention.
You will work in a supportive MDT, follow Care Act 2014 principles, and contribute to moving and handling guidance while maintaining robust records.
