Each Youth Practitioner covers a Geographical area within the Co Durham 0-25 Family Health Service and are very much part of the locality and County wide youth practitioner teams.

The Youth Practitioners work with young people aged 11–19 years old (25 for SEND) around Healthy lifestyles, with a strong focus on personal, social, emotional, cultural and educational development. They work with vulnerable young people both one to one and in groups delivering positive activities that develop self-esteem, confidence and resilience. Supporting them to develop healthy relationships and build Life skills.

Youth Practitioner referrals are received from within the 0-25 Family Health Service, Key Partners and from Young people and families themselves. A plan of intervention is jointly agreed with the Young Person with parental consent. These may include one to one wellbeing walks, engagement in the Growing Healthy Allotment Projects in the community and in schools. Young people will gain recognition for the positive contributions they make through the John Muir Award. The role will involve one to one appointments in schools and support with C Card and level 2 advice around smoking cessation. The youth practitioners have a good understanding of the local offer for Young People in the area they work and will signpost and support young people to engage with voluntary organisations and activities within the Community. The Youth Practitioners work in partnership with community organisations and services for young people and if there are identified gaps, they will actively participate in finding solutions. The Youth Practitioners also apply for grants to deliver specific tailored school holiday activities for the Young People they work with for example those experiencing social isolation, bullying, low level mental health problems and those engaging in activities that impact on their health.
