Overview
HDFT is seeking a motivated and passionate registered Nurse to join the innovative 0‑19 Service within the Stockton footprint, working as part of the Healthy Child Programme. The role will support the Public Health Nurse in planning, implementing, and evaluating a programme of care for children, young people and families across a blended approach that includes home visits, school settings and community venues.
Responsibilities
- Deliver contacts and targeted interventions as directed by the Public Health Nurse.
- Co‑ordinate, plan, implement and evaluate the Healthy Child programme for children and families.
- Support the completion of comprehensive health‑needs assessments and mobilise appropriate resources to meet identified needs.
- Take a proactive role in delivering national and Public Health agendas in response to identified health and social needs.
- Partner with other agencies, meet agreed standards, objectives and statutory requirements.
- Promote the physical, emotional and social development of children and engage with parents, carers, and partner organisations.
- Work as part of the children’s community health commissioned services alongside Public Health Nurses and other NHS staff.
- Operate within a hybrid model, working from a base closest to your home address and using software to work from home where appropriate.
Qualifications
- Registered Nurse with experience working with children and young people.
- Passion for providing high‑standard nursing care within a public health context.
- Excellent communication skills and strong organisational abilities.
- Flexibility to meet the needs of the service and independence to travel for service delivery.
- Ability to work collaboratively with Public Health Nurses and partner agencies.
