About the Role
As a Clinical Case Manager, you will play a vital role in ensuring our clients receive exceptional care tailored to their individual needs. You will be responsible for:
- Overseeing the clinical care of clients and ensuring high standards of service delivery
- Working closely with commissioners to take on new packages and support business growth
- Identifying and supporting staff to ensure continuity and quality of care
- Safeguarding clients, their families, and staff while maintaining compliance with CQC regulations
- Implementing and maintaining agreed care standards, reporting to the Senior Clinical Case Manager and Director of Nursing.
Key Responsibilities
- Assessing the physical and mental health needs of clients and creating tailored care plans
- Completing risk assessments and ensuring a safe care environment
- Acting as a key point of contact for external agencies, office teams, clients, and their families
- Conducting reviews to adapt care packages based on evolving client needs
- Providing training, supervision, and appraisals for healthcare staff
- Supporting recruitment, interviewing, and onboarding of healthcare professionals
- Responding to concerns and complaints in a professional and timely manner
- Participating in an on-call rota for out-of-hours support
What We’re Looking For
- A Registered Nurse with a valid NMC PIN
- A minimum of 5 years’ experience in health and social care, including community-based complex care
- Strong leadership, team management, and problem-solving skills
- Knowledge of CQC regulations and best practices in care provision
- Excellent communication, organisational, and IT skills
- A full UK driving licence and access to a vehicle
