As a Clinical Case Manager, you will oversee and manage MH/LD community care packages. You’ll work closely with commissioners, families, and support staff to ensure safe and effective service delivery, while promoting positive outcomes and independence for the people we support.
Key Responsibilities
- Lead the clinical oversight and management of MH/LD community packages.
- Liaise with commissioners and MDTs to assess and mobilise new packages of care.
- Develop person‑centred care plans that reflect the individual’s mental health and behavioural needs.
- Complete risk assessments and implement robust risk management strategies.
- Provide clinical leadership and guidance to support workers and RNs in the field.
- Conduct regular reviews and MDT meetings to ensure care remains safe and appropriate.
- Act as the main clinical point of contact for external stakeholders, families, and internal teams.
- Support staff through training, supervision, and clinical competency assessments.
- Participate in the on‑call rota to provide out‑of‑hours support as required.
- Ensure full compliance with CQC regulations and organisational governance standards.
What We’re Looking For
- Registered Mental Health Nurse (RMN) or Registered Nurse Learning Disabilities (RNLD) with a valid NMC PIN.
- Minimum 5 years’ experience in health and social care, with at least 2 years in community MH/LD services.
- Proven track record of managing complex community packages, including MDT involvement.
- Strong understanding of positive behaviour support, risk management and safeguarding frameworks.
- Excellent leadership, communication, and organisational skills.
- Knowledge of CQC regulations and best practice in MH/LD service provision.
- Full UK driving licence and access to a vehicle.
