Care Coordinator in Harlow

Harlow South Primary Care Network (PCN) has an opportunity for an experienced care coordinator to join our team on a 6 month fixed term contract, for 37.5 hours per week. This role has been created to support delivery of an exciting new project and ensures that patients receive coordinated and comprehensive care while maintaining high data quality standards, ultimately improving overall health outcomes.

This varied role will support coordination of the wide range of patient care that we offer, including Cancer Screening invitations, proactive long term care for high-risk patients and housebound appointment coordination with a focus on patients' journeys to meet their physical and/or mental health needs, working closely with the 3 practices in the primary care network (PCN). There will also be elements of back office and administrative support to ensure the smooth running of the PCN clinics.

Main duties of the job

• Provide extra time, capacity and expertise to our practice team to help us improve the care provided to our patients.
• Support the 6 month initiative to increase uptake in cancer screening programmes by proactively calling patients and increasing team capacity to deliver this project.
• Ensure patient health and care planning is timely, efficient, and patient-centred and that all patients receive the best possible care and service.
• Become an important member of your PCN multidisciplinary team (MDT) identifying patients in need of proactive support ensuring the appropriate support is made available to patients and their carers.
• Work alongside a variety of different people working in the practices and across the PCN and wider community teams including doctors, nurses, healthcare assistants, social prescribing link workers, physiotherapists, paramedics and clinical pharmacists, as well as the wider administrative team.
• Provide support to the operations lead to ensure the smooth running of the PCN clinics.
• Support patients to attend services in the PCN as an addition to their GP practice services.

Harlow South Primary Care Network is a friendly, supportive, well organised and enthusiastic PCN. GP practices are grouping together and working closely in what are called primary care networks (PCNs) and we are committed to providing traditional family orientated services, working together in the NHS to deliver better patient care on behalf of our 3 GP practices: Lister Medical Centre, The Hamilton Practice, and The Ross Practice. The PCN offices are based in a new purpose built Health Centre in Harlow. The PCN has over 40,000 patients from three practices. We are a training hub and support learning & development. We use the SystmOne clinical system.

Job responsibilities

• Provide high-quality care to patients, including telephone based assessments and follow-ups.
• Work collaboratively with social prescribing link workers, and other health professionals as needed.
• Maintain accurate and confidential patient records and documentation. Ensure the accuracy, completeness, and consistency of patient data.
• Participate in audits and data quality checks to identify and resolve discrepancies.
• Support the preparation and submission of monthly, quarterly, and annual claims and reports.
• Co-ordinate and minute Multi-Disciplinary Team Meetings (MDT).
• Invite and book reviews for a variety of patient appointments, including annual reviews.
• Provide signposting for Carers, e.g., to the social prescribing team.
• Take ownership for delivery in areas identified for improvement in patient care and implement solutions to enhance overall efficiency and effectiveness.
• Perform any other roles as deemed necessary to support the delivery of care to the Practice and PCN population.

The ideal applicant will have the following skills, experience and qualifications:

• Experience of working in a Primary Care setting, previous experience in a patient navigation or Care Coordinator role preferred.
• Strong written and verbal communication skills, able to communicate professionally and effectively with a variety of people at all levels.
• Good IT/computer skills including Word and Excel.
• Motivated by helping people with care and empathy and in delivering person centred support in a non-judgemental way.
• Awareness and understanding of confidentiality.

Computer Literacy

• Good IT/computer skills including Word and Excel.
• Experience working in SystmOne (TPP) in Primary Care.
• Experience running searches and reports, and setting up clinics and rotas in SystmOne.

• Have an awareness and understanding of confidentiality in a clinical setting.
• Information Governance knowledge.

Experience

• Experience in a Patient facing role in the NHS.
• Experience of working in Primary care, GP or a PCN setting.
• Strong written and verbal communication skills, able to communicate professionally and effectively with a variety of people at all levels.
• Able to work effectively as part of a multidisciplinary team.
• Ability to communicate complex ideas in a concise and understandable way.
• Experience communicating with patients regarding cancer screening.
