This is a community-based role with travel across Northern Ireland. No two days are the same, and every visit makes a genuine difference to patients and families.
Key Responsibilities
- Develop and review person‑centred care plans
- Risk assessments and clinical decision‑making
- Medication management and prescribing (if qualified)
Complex Care Delivery
- Tracheostomy and ventilation care
- PEG feeding and IV therapy
- Complex wound management
- Palliative and end‑of‑life care
- Supervise and mentor care staff
- Clinical delegation and oversight
- Competency assessments for carers
- Support induction of new staff
Governance & Compliance
- NMC Code of Conduct adherence
- RQIA standards and inspections
- Safeguarding and incident reporting
- Clinical audit and quality improvement
Patient & Family Support
- Build therapeutic relationships
- Educate patients and families
- Coordinate with MDT professionals
- Advocate for patient needs
Specialist Knowledge
- Dementia and cognitive care
- Chronic disease management
- Health promotion and prevention
- Active NMC registration (Adult, Mental Health, LD, or Children’s Nursing)
- Minimum 2 years post‑registration experience
- Community nursing or domiciliary care experience
- Complex care competencies (tracheostomy, PEG, IV, ventilation)
- Full UK driving licence and access to a vehicle
- Right to work in the UK
- Excellent clinical assessment and documentation skills
- Strong communication and interpersonal skills
- Prescribing (V100/V150/V300)
