Harley House Supported Living is seeking a Transition Manager to coordinate referrals and transitions across supported living provisions in the north‑east Essex, Ipswich and Norwich. This part‑time role spans three days (22.5 hours per week) with a full‑time equivalent salary of £40,000.
You will report to the Head of Care and work with the HR and Operations teams to ensure smooth handovers, robust assessment processes and continuity of care for service users.
