About the Role: As the Continuing Health Care Team Manager, you will provide strategic and operational leadership to a newly formed, specialist CHC team within Adult Social Care. This is a pivotal role ensuring the delivery of high quality, lawful and person centred Continuing Health Care assessments, reviews and integrated casework in line with the National Framework for NHS Continuing Healthcare and NHS funded Nursing Care. You will lead the day to day operation of the CHC service, overseeing eligibility assessments, fast track processes, reviews and complex decision making. You will ensure practice is strengths based, outcome focused and legally compliant, with a strong emphasis on defensible decision making, robust risk management and clear professional oversight. Working closely with NHS colleagues, Integrated Care Boards (ICBs), health providers and internal council teams, you will play a central role in maintaining effective multi agency pathways and resolving complex funding and discharge challenges. You will lead engagement with panels and contribute to strategic discussions to ensure residents experience timely, coordinated and fair access to CHC processes.

About the Team: Haringey’s Continuing Health Care Team is a newly established, specialist service within Adult Social Care, created to strengthen and embed high‑quality CHC practice across the borough. The team is made up of skilled practitioners who work at the interface of health and social care, supporting residents with complex, long‑term health needs. We work in close collaboration with NHS partners, providers and internal services to deliver consistent, transparent and person‑centred CHC processes.

About You: You will be an experienced, values led adult social care professional with strong leadership capability and in depth knowledge of Continuing Health Care. You are confident managing complexity, influencing across organisational boundaries and supporting practitioners to deliver high quality outcomes while maintaining strong governance and accountability. You bring resilience, excellent communication skills and a collaborative approach, enabling you to build trusted relationships with health partners and confidently represent the local authority within multi agency forums and panels.

  • Qualified Social Worker with current Social Work England registration
  • Significant experience working within statutory adult social care, ideally within a local authority setting
  • Proven experience of leading and developing teams delivering complex casework, including supervision, performance management and staff development
  • In depth knowledge of the CHC National Framework, relevant legislation and multi agency pathways
  • Confidence in overseeing complex, high risk cases, panels and dispute processes, ensuring defensible decision making and effective risk management
  • Excellent communication, negotiation and partnership working skills, particularly with NHS colleagues and ICBs
  • Experience of performance management and quality assurance, including audits, data analysis, service standards and continuous improvement
