Utilise and grow awareness of relevant clinical pathways and local childrens services. Access and update patient records in a Haringey community EMIS, escalating issues accordingly to prevent delays to patient care. Prepare updates on patient cases base on consultations relating to the Super Triage MDT, ensuring that information is presented in a concise and streamlined way. Complete an EMIS template for each case discussed in the MDT.
Follow through on clinical actions assigned to the Lead GP from the Super-Triage MDT and other meetings, expected ~48-hour turnaround time on actions. Communicate and relay relevant actions to the home GP practice resulting from Super-Triage discussion. Note and share learning points discussed from case discussion in the Super-Triage MDT. Distribute shared learning to relevant stakeholder organisations at Neighbourhood, PCN, and Practice platforms if areas of educational need are identified.
Contribute to the production of materials used within paediatric neighbourhood systems, including but not limited to appointment infrastructure resources such as scripts and processes, or key learning points arising from case discussion. Provide monthly reporting on activity and outcomes from the Super-Triage MDT alongside the operational lead of the service. This set of primary responsibilities is not exhaustive of all areas of the job and is likely to evolve over time.
