We are looking for a dedicated and passionate Health & Wellbeing Coach delivering on our One You Haringey smoking cessation services. You will play a vital role in helping individuals improve their overall health and well-being.
The Role Will Include:
- Delivering stop smoking clinics and offering person-centred support to residents who want to quit smoking.
- Providing ongoing support and encouragement to participants throughout their journey with the service.
- Delivering stop smoking sessions face to face within community settings, and remotely by telephone.
- Monitoring, reviewing and evaluating participant progress against agreed outcomes, adjusting support plans as required.
- Collaborating with other healthcare professionals and community organisations to promote the One You Haringey programme.
- Maintaining accurate, timely and confidential records of client interactions and program outcomes.
- Liaising with professionals including GPs and primary care staff to promote and generate referrals to the service.
- Attending community outreach events to promote the service within Haringey.
- Contributing to the achievement of service targets and performance outcomes.
About Us:
Haringey GP Federation is a dynamic and growing organisation working in partnership with GP practices across the diverse borough of Haringey, north London. Formed in 2016 by local GPs, we now work alongside over 30 practices within seven Primary Care Networks to help deliver high-quality, accessible healthcare to our local communities.
Job responsibilities:
Working Days For This Role Are: Saturday mornings; Thursdays in our office in Wood Green; 1 other day which will include an evening; The role is hybrid.
Person Specification:
Qualifications:
- Essential: A minimum of 2 A-Levels or an equivalent Level 3 Diploma; GCSE English Language at a minimum of a Grade B (or equivalent Level 2 qualification) OR achievement of a Level 2 in Literacy Assessment at interview stage.
- Desirable: Degree in Public Health, Psychology, or related subject; NCSCT Level 2.
Skills and Abilities:
- Excellent customer service skills.
- Strong verbal communication skills.
- Good influencing skills.
- Highly organised with an ability to work to targets.
- Ability to follow processes to ensure provision is made for residents to access and complete lifestyle interventions.
- Attentive to detail, ensuring all relevant data is captured accurately.
Experience:
- Essential: Experience of working effectively in a team; Demonstrable experience of delivering high quality customer service; Experience of working in a fast-paced environment; Experience of using database systems to input data and track progress of customer/potential customers.
- Desirable: Previous experience of delivering smoking cessation interventions.
Knowledge:
- Essential: Knowledge of evidence-based life-style interventions.
- Desirable: Knowledge of borough health needs and fluent in languages common in the borough (e.g., Turkish, Polish, Spanish, Somali, Portuguese, Greek) in addition to English.
