Main duties of the job:

North Central London Integrated Care System partners in Haringey are working together to redesign how diabetes care is delivered for local residents. Although people living with long-term conditions spend less than 1% of their time in contact with health care services, self-management support has historically not been prioritised despite strong recommendations by NICE guidance.

To strengthen support for residents, Haringey is piloting My Way Diabetes, a digital self-management platform designed to help people living with diabetes build the knowledge, skills, and confidence to manage their conditions more effectively. The platform provides more personalised information, educational resources and tools that enable residents to better understand their diabetes and take a more active role in their day-to-day health.

This pilot forms part of Haringey's wider commitment to improving outcomes, reducing inequalities and empowering residents through accessible, evidence-based self-management support.

The service will act as a single point of access for diabetes self-management in this neighbourhood and the health coach will be an integral part of this new and exciting approach to working with people with diabetes.

The post holder will work with the individual, MyWay Diabetes (a self-management platform) plus primary, community, prevention, VCSE and secondary care services improving self-efficacy/activation and engagement.
