As the Continuing Health Care Team Manager, you will provide strategic and operational leadership to a newly formed, specialist CHC team within Adult Social Care. You will ensure the delivery of high‑quality, lawful and person‑centred Continuing Health Care assessments, reviews and integrated case‑work in line with the National Framework for NHS Continuing Healthcare and NHS funded Nursing Care. You will lead the day‑to‑day operation of the CHC service, overseeing eligibility assessments, fast‑track processes, reviews and complex decision‑making, while maintaining strong governance and accountability.

Responsibilities

  • You will work closely with NHS colleagues, Integrated Care Boards (ICBs), health providers and internal council teams to maintain effective multi‑agency pathways, resolve complex funding and discharge challenges, engage panels, and contribute to strategic discussions to ensure residents receive timely, coordinated and fair access to CHC processes.

Qualifications

  • Qualified Social Worker with current Social Work England registration
  • Significant experience working within statutory adult social care, ideally within a local authority setting
  • Proven experience of leading and developing teams delivering complex casework, including supervision, performance management and staff development
  • In depth knowledge of the CHC National Framework, relevant legislation and multi‑agency pathways
  • Confidence in overseeing complex, high‑risk cases, panels and dispute processes, ensuring defensible decision‑making and effective risk management
  • Excellent communication, negotiation and partnership working skills, particularly with NHS colleagues and ICBs
  • Experience of performance management and quality assurance, including audits, data analysis, service standards and continuous improvement
