About the Role
We are seeking experienced and motivated Specialist Continuing Healthcare (CHC) Social Workers to join our developing and dynamic CHC service. This is an exciting opportunity to be part of a newly established specialist team, playing a key role in delivering high-quality Continuing Healthcare assessments and decision‑making. You will work closely with ICB Nurse Assessors to complete Decision Support Tools (DSTs), and undertake Care Act assessments, reviews and support planning for adults with complex and often rapidly changing needs. The role requires strong partnership working across multi‑disciplinary teams to ensure individuals receive appropriate, timely, and person‑centred care packages, with clear alignment between health and social care responsibilities. You will manage a complex caseload, providing robust analysis of needs across CHC domains, contributing to eligibility decision‑making, and ensuring care packages are reviewed and remain proportionate and effective. A key part of the role is to ensure CHC processes are delivered in line with the National Framework for NHS Continuing Healthcare, supporting lawful, consistent, and defensible decision‑making while maintaining a strong focus on outcomes for residents.
About You
We are looking for a proactive and knowledgeable practitioner who can bring both expertise and a collaborative approach to this specialist role.
You Will Have:
- Current registration as a Social Worker with Social Work England (SWE)
- Significant experience working with adults with complex, long-term, or unpredictable health and care needs
- Strong working knowledge of the Care Act 2014, Mental Capacity Act, safeguarding frameworks, and the National CHC Framework
- Proven experience completing or contributing to CHC assessments and DSTs, with the ability to analyse evidence across domains
- Excellent skills in strengths‑based assessments, care planning, and case management
- Confidence working within multi‑disciplinary settings, influencing decision‑making and advocating for service users
- Strong analytical and recording skills, with the ability to produce clear, evidence‑based reports
- The ability to manage competing priorities, work autonomously, and maintain high professional standards
- A commitment to person‑centred practice, equity of access, and continuous service improvement
