We are seeking an experienced and motivated Social Worker to join our Hospital Discharge Team.

The successful candidate will undertake person‑centred assessments and reviews for Haringey residents, service users, and their carers in accordance with the Care Act 2014.

Responsibilities

  • Complete Care Act assessments, reviews, and support planning.
  • Promote independence and wellbeing through a strengths‑based approach.
  • Identify and implement preventative interventions to reduce, delay, or avoid the need for ongoing support.
  • Work collaboratively with health professionals, voluntary organisations, community groups, service users, and carers to achieve positive outcomes.
  • Support safe and timely hospital discharges while ensuring statutory responsibilities are met.
  • Manage a complex caseload within a fast‑paced hospital environment.

Qualifications

  • Qualified Social Worker with Social Work England registration.
  • Significant experience of working within a hospital setting and supporting hospital discharge pathways.
  • Sound knowledge of the Care Act 2014, Mental Capacity Act, and safeguarding legislation.
  • Experience of undertaking strengths‑based assessments and person‑centred support planning.
  • Ability to work effectively within a multidisciplinary team and manage competing priorities.
  • Working Arrangements

This is a hybrid role, with an expectation that staff will work on‑site at least three days per week, alongside flexible remote working arrangements where service needs allow.

Hospital experience is essential for this role and applications from candidates without recent hospital‑based social work experience will not be considered.
