Care Coordinator or Senior Care professional
looking for a varied role with the opportunity to work across several different services? We are recruiting for a Care Coordinator to support a growing organisation providing
domiciliary care, children's respite, out-of-school support and alternative provision
across Thurrock. The services currently support
over 50 children and a small number of adults , providing a combination of personal care, companionship and specialist support. The role
You'll help coordinate the day-to-day delivery of services, including: Managing staff rotas and ensuring appropriate cover
Coordinating care and support packages
Maintaining care plans and risk assessments
Liaising with families, social workers, schools and other professionals
Supporting staff supervision and monitoring service delivery
Responding to changes in care or staffing requirements
Supporting safeguarding and compliance
Maintaining accurate records
You'll also support the organisation's
alternative provision in Thurrock , working with children and young people who are unable to access mainstream or SEN education. About you
We're looking for someone with: Previous care coordination, senior care or similar experience
Experience supporting children and/or young people with additional or complex needs
Level 3 in Health & Social Care, Children & Young People or similar
Experience with rotas, care planning and risk assessments
Good safeguarding knowledge
Strong organisation and communication skills
Confidence working with families and external professionals
Experience within
children's services, domiciliary care, respite or alternative provision
would be particularly valuable. Hours & pay
