Harbour Healthcare are recruiting for a Care Home Assistant Practitioner (CHAP) to join our dedicated team at Riverside Court Care Home. This is an exciting opportunity for an experienced care professional who is passionate about delivering exceptional care and wants to develop advanced clinical skills within a supportive and rewarding environment. What You’ll Be Doing

  • Supporting Registered Nurses in delivering outstanding resident care
  • Undertaking delegated clinical procedures including observations, wound care, skin assessments and nutritional support
  • Assisting with medicines management and administration
  • Monitoring residents’ wellbeing and escalating concerns appropriately
  • Supporting care planning and ensuring person-centred care is delivered
  • Supervising and mentoring care colleagues
  • Maintaining accurate records and contributing to quality improvement initiatives
  • Promoting dignity, independence and respect at all times

What We’re Looking For

  • Minimum 12 months’ experience within a Nursing Home, Care Home or Hospital setting
  • Level 2 Health & Social Care qualification as a minimum (Level 3 desirable)
  • Excellent communication and interpersonal skills
  • A caring, compassionate and professional approach
  • Leadership potential and the ability to support colleagues
  • A willingness to complete Harbour Healthcare’s bespoke CHAP training programme
