Case Manager (Aftercare & Client Pathways)Part-time or Full-time | London (hybrid) | Reporting to: Clinical Director
Harbor London is a premium mental health and addiction treatment service. We are seeking a Case Manager to coordinate step-down and aftercare delivery for clients and families. This is a less clinical role than a therapist post – highly organised, discreet, and client-facing.
What you will do
Coordinate the client pathway from in-treatment planning through step-down and aftercare; keep weekly delivery on track.
Schedule and coordinate MDT input and external providers; manage actions, follow-ups, and high-quality handovers (with consent).
Support structured family communication and planning; elevate clinical concerns promptly via Harbor governance.
Maintain clear documentation and contribute to improving aftercare workflows as the service grows.
What you will bring
Experience coordinating complex client journeys in private healthcare, rehab, mental health/addiction services, care navigation or UHNW services.
Strong MDT and stakeholder management skills; excellent organisation, boundaries, and calm under pressure.
High discretion and communication discipline (written and verbal); flexibility for occasional evenings/weekends by arrangement.
Private-sector high-touch experience; familiarity with aftercare/step-down models; international coordination exposure.
