Harbor London is a premium mental health and addiction treatment service. We are seeking a Case Manager to coordinate step-down and aftercare delivery for clients and families. This is a less clinical role than a therapist post - highly organised, discreet, and client-facing.
What you will do:
- Coordinate the client pathway from in-treatment planning through step-down and aftercare; keep weekly delivery on track.
- Schedule and coordinate MDT input and external providers; manage actions, follow-ups, and high-quality handovers (with consent).
- Support structured family communication and planning; elevate clinical concerns promptly via Harbor governance.
- Maintain clear documentation and contribute to improving aftercare workflows as the service grows.
What you will bring:
- Experience coordinating complex client journeys in private healthcare, rehab, mental health/addiction services, care navigation or UHNW services.
- Strong MDT and stakeholder management skills; excellent organisation, boundaries, and calm under pressure.
- High discretion and communication discipline (written and verbal); flexibility for occasional evenings/weekends by arrangement.
- Private-sector high-touch experience; familiarity with aftercare/step-down models; international coordination exposure.
