Join Harbor Health Services, Inc. as a Nurse Practitioner Per Diem. We are seeking qualified Nurse Practitioners to join our Primary Care and Specialty teams at our health centers in Boston, Plymouth, and Hyannis. We offer day, evening, and weekend shifts.
Locations:
- Daniel Driscoll-Neponset Health Center, Dorchester (Family & Adult Medicine, Women's Health & Pediatrics)
- Geiger Gibson Community Health Center, Dorchester (Family Medicine, Primary & Same Day Care)
- Harbor Community Health Center, Plymouth (Family Medicine)
- Harbor Community Health Center, Hyannis (Family Medicine)
Responsibilities:
- Provide outpatient care, including routine health maintenance, management of chronic and acute illnesses, and referrals for specialty care.
- Collaborate with physicians and healthcare team members.
- Participate in meetings, trainings, and quality improvement initiatives.
- Ensure accurate documentation in EMR and maintain current patient records.
Minimum Requirements:
- Graduation from an accredited Nurse Practitioner program in Adult Medicine, Family Practice, Pediatrics, or Women's Health.
- Current Massachusetts NP license, Board Eligible/Certified, DEA, MCSR, and BLS certifications.
- Proficiency in MS Office and EMR systems (EPIC preferred).
- Experience with community health center populations is preferred.
