Harbor Community Health Center in Plymouth is seeking a licensed behavioral health provider to join their team.

About Harbor Health Services: Harbor provides medical, behavioral health, dental, and support services to over 34,000 patients across Boston, the South Shore, and Cape Cod. Position Overview: The Behavioral Health Provider for Primary Care (LICSW, LMHC, or LFMT) will deliver psychotherapy services, support clinical functions, and collaborate with primary care teams. This role is based at the Harbor Community Health Center in Plymouth, a federally qualified health center (FQHC), eligible for federal and state loan repayment programs.

Responsibilities:

  • Conduct intakes, evaluations, and therapy sessions with assigned patients.
  • Participate in primary care and behavioral health treatment teams, providing brief therapies, crisis management, and consultations.
  • Obtain insurance authorizations and track visits.
  • Complete assessments, treatment updates, and discharge summaries.
  • Coordinate with psychiatry and primary care providers for integrated services.
  • Manage collateral case work, including disability forms and consultations.
  • Facilitate trainings, case conferences, and conduct utilization reviews.
  • Provide supervision to behavioral health staff and interns as directed.
  • Attend case conferences and in-service trainings.

Qualifications:

  • Current MALICSW, LMHC, or LMFT license.
  • Master's Degree in relevant discipline.
  • At least 2 years of therapy experience preferred.
  • Current Massachusetts LADC1 preferred.
  • One year of substance abuse treatment experience preferred.
