The Role

As an Independent Living Assessor, you can make your own powerful contribution to people across Westminster. Joining our Community Independence Service, you’ll play a vital role in supporting adults with complex health and social care needs to live safely and independently at home. Responsible for carrying out assessments and reviews of service users’ functional abilities, you’ll set goals, coordinate support and monitor the effectiveness of packages of care — all with the goal of empowering service users and maximising their independence.

Working collaboratively alongside colleagues, partner agencies and the NHS, you’ll signpost other services that might be beneficial to customers, liaising and negotiating on their behalf to guarantee they get the best service possible. Where needed, you’ll arrange for the provision of specialist equipment, assistive technology and adaptations, ensuring that customers are able to carry out daily activities safely and reducing dependence on statutory services.

A proactive and empathetic professional, you’ll have some knowledge of the conditions that can lead to a disability, the impact disability has on individuals, and the range of services available to people with disabilities. An understanding of the essential role of care coordination and Trusted Assessor within a social care/ health setting is desirable, as is knowledge of anti-discriminatory practice and the needs of diverse communities.

On a personal level, you’ll be a true team player with excellent time management, prioritisation and written and verbal communication skills. Hungry to learn and committed to the development of both your own skillset, and the service as a whole, you’ll have the ability to vary your communication style to clearly and accurately convey information to users, colleagues, carers and managers.
