The Role:
As Placement Monitoring Officer, you will manage a caseload of adults with complex needs, regularly ensuring that their service plans are fit-for-purpose. You will monitor the health and social care aspects of all Support Plans, ensuring that a good standard of care is being provided. Our service users are in residential placements, nursing homes and supported living facilities. You will establish relationships that promote trust and confidence, give each service user clear information about their rights and responsibilities, and work with them to agree on a support plan with measurable outcomes that maximises their independence.
You will carry out holistic assessments, care checklists and reviews into finances. Risk assessments will reflect a person’s capacity to make decisions and acknowledge that they have the right to take risks. If you’re ever concerned about abuse of a vulnerable adult, you will undertake Safeguarding Adults Investigations and report this in line with policy. You will prescribe equipment, assistive technology and minor adaptations to maximise an individual’s independence. Another aspect of your role will be liaising with Social Services to ensure that placements meet service specifications.
To be suitable for the role, you will already be familiar with the legislation, statutory guidance and best practice that underpins social work and care for adults and health. You can take laws and official guidance and turn them into clear, workable policies and day-to-day practice. You have constructed measures, metrics and performance indicators to shape and drive performance. We are looking for a highly experienced people manager, capable of working with staff and others to embed organisational values.
