The Role:

Joining us as a Senior Case Worker, you’ll be part of our friendly and supportive team of social care and health care professionals based at Basingstoke and North Hampshire Hospital. You’ll also support our short-term service to deliver safe and efficient hospital discharges. You’ll enjoy the benefits of engaging with people in a face‑to‑face environment, working alongside colleagues in a supportive space. There will be plenty of opportunities to collaborate and share learning with peers, whilst building positive relationships through your reviews in the hospital.

What you’ll be doing:

  • Completing assessments and reviews of individuals’ care and support needs in Basingstoke and North Hampshire Hospital, local community hospitals and care homes.
  • Providing support and guidance to individuals to maximise their wellbeing and independence using a strengths‑based approach.
  • Collaborating with individuals, their families, providers, and other healthcare professionals to find and signpost to innovative, alternative community resources to meet individuals’ needs.
  • Having the confidence and knowledge to make well‑informed decisions where appropriate, as well as knowing when a case or situation requires escalation to senior colleagues.

  • Strong communication, negotiation and relationship management skills.
  • Ability to work with people in difficult circumstances in a compassionate and collaborative way.
  • Strong analytical and problem‑solving skills.
  • Sound IT and record‑keeping skills.
  • Ability to work independently as well as part of a team.
  • Knowledge of the Care Act 2014 and its eligibility criteria.
  • Knowledge of the Mental Capacity Act 2005 and Best Interest decision making.
